 Hillsborough County Y.E.S. Volunteer Form 

 This form is REQUIRED by Hillsborough County Public Schools to be completed by anyone who volunteers at the school or serves as a chaperone for off campus school activities such as class field trips.  This form must be completed by both NEW and RETURNING volunteers and must be renewed every year  PRIOR to beginning volunteer activities.  Returning volunteers should have been issued a PIN number that can be used to retrieve information from the previous year's application that will speed up the reapplication process.
